CHN Diabetes Education
The overall prevalence of diabetes in Wisconsin and, specifically, the Central Wisconsin area (Green Lake, Adams, Waushara and Marquette Counties) has increased dramatically...as much as 27%! Diabetes has reached epidemic levels in Wisconsin; 9.6% of all adults and 6,000 children and adolescents have diabetes and the number is rising.
Related societal costs have increased substantially as well. Diabetes-related hospitalizations increased nearly 11%, diabetes-related hospitalization charges increased more than 48%, and the overall estimated costs of diabetes in adults has increased 16% from $4.47 billion to $5.19 billion.
These dramatic increases demonstrate how important it is for each of us to renew our commitment to prevent, and increase the management of, diabetes in Wisconsin.

NANCY GIMENEZ, RN, BSN, CDE
Certified Diabetes Educator
Nancy attained her Bachelor of Science in Nursing from UW-Oshkosh in 1995. She joined CHN in 2001.
DANIELLE KRUEGER, RD
Danielle has a Bachelor of Science degree from Viterbo College in LaCrosse, WI. She has worked at Berlin Hospital since 2002 as a Clinical Dietitian.
KATHLEEN LEGARE, RD
Kathleen is a graduate of the University of Wisconsin – Green Bay. She joined CHN in December, 2011.
AMY SCHROEDER, BSN, APNP
Family Nurse Practitioner
Amy graduated from Winona State University with a Bachelor of Science in Nursing, and later attended University of Wisconsin – Oshkosh for a Masters degree. She has been working for CHN since 2005.
MELANIE SIMPKINS, RN, BSN, MPH, FCN
Melanie attained her Bachelor of Science degree from St. Olaf College and Masters in Public Health from University of Minnesota. She started working at CHN in June, 2012.
